Admission Criteria

  • Cabaniss admits only those individuals whose service needs are consistent with the service description, for whom services are available, and for which staffing levels and types meet the needs of the individuals served. Individuals must have a diagnosis of mental retardation (intellectual disability), mental retardation/mental illness or a developmental disability and must be at least thirteen (13) years of age.
  • The referring agency completes a Cabaniss Day Support, Inc. application on behalf of the individual who wants to receive day support services. The individual who meets the minimum admission criteria, will meet with Cabaniss staff and complete a preliminary Skills Behavior Assessment to confirm that the individual qualifies for admission and to develop a preliminary individualized services plan.
  • The applicant will be invited to spend time in the Day Support program. Requests for funding are then made by the Community Services Board or other referring agency. An assessment period usually occurs over a sixty (60) day period to assure that the Day Support staff is thoroughly familiar with the needs of the individual and that they and/or their family are satisfied with the identified service. During these 60 days, an Assessment of Adaptive Abilities (AAA), a Functional Independence Skills (FISH) assessment and /or the Becker Work Adjustment (BWA) Profile will be completed.
